What should I Bring?
Bring the following items to your appointment:
- Photo ID
- Health Insurance Card/Information
- Medication List

A copy of your X-Rays/CT scans
(not just the reports, get the actual films or cd) - Completed FORMS (see patient forms)
- List of your questions/concerns
- A friend or family member to take notes/help with questions

You will then meet one of our specialists. A head and neck exam will be performed, and, if needed, may include a specialized nose and throat exam called an endoscopy. During this minor procedure, a small endoscope is used to evaluate the internal structures of the nose and throat. Your provider may also order a hearing exam if needed.

Finally, our staff will assist you with obtaining any prescriptions and scheduling any tests, surgeries, and additional appointments. If further questions arise after leaving the office, feel free to call back and get them answered. Results will be sent to your primary care provider in a timely manner, detailing and summarizing your visit.
Financial Responsibility
Our office participates with most insurance plans. Please remember that insurance is considered a method of reimbursement and is not a substitute for payment. Some insurance companies pay fixed allowances for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance, or any other balance that is not paid by the insurance. In order to control our cost of billing, we request that you pay your portion of the bill at the time of each visit. We accept cash, check, Visa, Discover, MasterCard, and CareCredit.
